Obituary

At his request, there will be no service for Andrew O. Hayworth, who died Sept. 24, 1996, of cancer at age 33.

Mr. Hayworth was born Aug. 2, 1963, in Phoenix, Ariz., and lived in Portland 15 years. He attended Portland Community College. He worked as a department store cashier and customer service representative.

Survivors include his mother, May Williams of Gervais; brothers, Dale B. of Portland, Robert W. of Beaverton, Larry D. of Salem, Preston C. Blanton of Phoenix, Ariz., Jeffry Blanton of Massachusetts and Hank Williams of Gervais; sisters, Ella M. Thompson of Tempe, Ariz., Edna F. King of Prairie Grove, Ariz., Janice Martinez of Salem, Wanda Bettis of Portland, Mary Ford of Vancouver, Wash., Carol Kelly of Yakima and Jewell Shores of Silverton.

Disposition was by cremation. Arrangements are by Autumn Burial & Cremation.
